How to get CGMS reimbursement

United Health Care (UHC)

Plan is the POS (but next year changing to high deductible plan with pre-tax Health Savings Account (HSA) to cover $4500 deductible then 100% covered.)

Group# is 226310: Delta Airlines Employee - Self-Insured Employer

MM looked into my insurance (UHC) and quickly came back that it is covered after my deductible at 80%.

UHC didn't seem to have any trouble saying it was covered. This includes the pump upgrade and all the sensors.

The problem I have is that MM says the pump upgrade is about $5000 minus $500 trade-in allowance. Then the CGMS is $1000 - which includes 30days of sensors. Since I have $200 to meet on my deductible the out of pocket cost is about $800.

All of my Pump supplies/diabetic supplies are covered 100% if I get them in a 3 month kit...that does not yet include the CGMS sensors and no longer includes insulin."

cap

well my uhc has a cap on dme supplies so it will be very expensive for me to get the minimed pump and sensor they cover it but only 2500 which isn't much on this the sensors are 350 a month that is just for the sensors not the infusions i haven't even checked that part out yet i have a cozmo and it looks like for now that is what i am stuck with i have low bs unawareness and can't do much since i have two young children i don't want to drive and have a problem my bs drops fast sometimes.

self insured companies

I work for Harley Davidson in the factory, and currently having to appeal to the company since they are self insured and Blue Cross Blue shield has denied the inital request and the first appeal. I was orginally denied a new pump, they said it was not a medical necessity and I have been on a pump for over 8 years. I didn't think I was asking for much.

Glad to hear you had very little problems, hope to find some wording to use in my appeal process.

United Health Care PPO

UHC PPO (Executive Plan, San Diego) covered my daughter's CGMS. The plan paid 100% for the initial starter kit. It now pays 90% for sensor scrips.

The scrip was paid on initial request...no appeal was needed.
